In order to use online repeat prescription requests you will require a username and password. Please collect an online application form from reception.
All prescriptions will be checked by a doctor. Medications are also reviewed (usually every six months) by your doctor. If your medication review date is within the next month, or has passed, please let the surgery know (at the bottom of the form) and we will let you know if you need to see a doctor or nurse in order to reauthorise your medication.
