HOW TO REGISTER
Welcome to Saxmundham Health
If you live within our Practice catchment area you are welcome to register with us and our Reception staff will be happy to guide you through the procedure. Eligibility can be quickly confirmed from your address so it would help if you could provide proof by way of a recent utility bill.
New Patient Pack
We will soon have a 'Patient Pack' available. New patients can use it to register with the Practice and it contains a wealth of information about our services and how to access healthcare. A hard copy is available from Reception or, coming shortly, a downloaded PDF version.
Click on the links below to print off both a GMS1 registration form and a health questionnaire; complete both and bring to the practice as soon as possible. If you've received NHS care before, please supply your NHS Number; you may be able to get with the Find my NHS Number service, from your previous GP surgery or on NHS letters or prescriptions you already have.

Adult / Child / Newborn
All new patients are required to fill in a registration questionnaire at Reception and provide proof of previous prescriptions within three months of registering. This enables us to establish a basic medical record on our computer system, whilst we wait for your medical records to arrive from your last GP.
We require proof of previous prescriptions to enable us to organise your future medication requirements. You will be provided with a months of medication after which you will be required to come in for a medical review.
Out of Practice Area Registration
A new patient registration arrangement was introduced from January 2015 that gives us the ability to accept patients who live outside the practice area without the obligation to provide home visits. For us to ensure your clinical care is safely managed and that you can receive a seamless and full range of services, it is our policy to continue not registering patients who live outside our advertised practice area.
